Furtrethonium (furfuryltrimethylammonium iodide) is a synthetic quaternary ammonium compound structurally related to muscarinic agonists such as methacholine. The furfuryl (furan) substituent confers increased lipophilicity compared to acetylcholine. It has been used in ophthalmic formulations to stimulate tear secretion and ciliary body activity in dry eye conditions, and in pharmacological research as a muscarinic probe. The quaternary ammonium structure prevents CNS penetration but allows peripheral muscarinic effects.
